Esistono più di 7.000 malattie genetiche conosciute. Nessun medico può conoscerle tutte: serve qualcuno che sappia dove cercare, e come.

There are more than 7,000 known genetic diseases. No doctor can know them all: you need someone who knows where to look, and how.

Gli esami invecchiano

Tests get old

Un esoma "negativo" di tre anni fa è stato letto con le conoscenze di tre anni fa. Ogni mese nuovi geni vengono collegati a nuove malattie: gli stessi dati, oggi, possono dire di più.

A "negative" exome from three years ago was read with the knowledge of three years ago. Every month new genes are linked to new diseases: the same data, today, can say more.

Nobody puts the pieces together

Il neurologo vede il cervello, il cardiologo il cuore, il laboratorio vede i dati. Il genetista clinico è quello che mette tutto sullo stesso tavolo e cerca la spiegazione unica.

The neurologist sees the brain, the cardiologist the heart, the laboratory sees the data. The clinical geneticist is the one who puts everything on the same table and looks for the single explanation.

Sono Marco Cassone, pediatra e genetista a Modica.

I am Marco Cassone, paediatrician and geneticist in Modica.

Da anni seguo famiglie che hanno girato reparti e laboratori senza che nessuno desse un nome a quello che hanno. Ho scelto di fare il genetista dopo aver fatto il pediatra, e di imparare a leggere io stesso i dati genomici invece di affidarli a un programma: perché una variante ha senso solo se qualcuno la guarda insieme al bambino che ha davanti. Lavoro a Modica e, in videochiamata, con famiglie di tutta Italia.

For years I have followed families who went from ward to laboratory without anyone naming what they have. I chose to become a geneticist after being a paediatrician, and to learn to read genomic data myself rather than hand it to a program: because a variant only makes sense if someone looks at it together with the child in front of them. I work in Modica and, by video call, with families across Italy.

In studio a Modica o in videochiamata. Ricostruiamo l'albero familiare, mettiamo in fila i sintomi con il linguaggio che usano i genetisti di tutto il mondo (HPO) e decidiamo insieme cosa ha senso fare.

At the Modica practice or by video call. We rebuild the family tree, line up the symptoms in the language geneticists use worldwide (HPO) and decide together what makes sense to do.

Rileggo i dati che avete già, oppure, se serve, indico l'esame giusto da fare. L'analisi la faccio io, con la mia piattaforma gETIKA, su doppio riferimento genomico: non la delego a un algoritmo generico.

I re-read the data you already have or, if needed, indicate the right test to do. I do the analysis myself, with my gETIKA platform, on a dual genomic reference: I don't delegate it to a generic algorithm.

Ne parliamo insieme. Se c'è una diagnosi, cosa comporta per il paziente e per i familiari. Se non c'è, cosa si può ancora fare e quando ha senso riprovare.

We talk it through. If there is a diagnosis, what it means for the patient and relatives. If there isn't, what can still be done and when it makes sense to try again.

Troverete sicuramente la causa?Will you definitely find the cause?

No, e chi ve lo promette non è onesto. Con esoma e genoma letti bene, una parte importante delle famiglie ottiene una risposta; le altre ottengono almeno la certezza di aver cercato nel modo giusto, e un piano per quando le conoscenze cresceranno. Se penso che le probabilità siano basse, ve lo dico prima, non dopo.

No, and anyone promising that is not being honest. With exome and genome read properly, a significant share of families get an answer; the others at least get the certainty of having searched the right way, and a plan for when knowledge grows. If I think the odds are low, I tell you before, not after.

Serve un nuovo prelievo?Is a new blood sample needed?

Se avete già fatto un esoma o un genoma, quasi mai: i dati sono vostri e il laboratorio deve darveli. Se non avete mai fatto esami genetici, il prelievo si fa dopo la consulenza, una volta deciso l'esame giusto.

If you have already done an exome or genome, almost never: the data is yours and the laboratory must give it to you. If you have never done genetic tests, the sample is taken after the counseling, once the right test is decided.

Tutto il percorso può svolgersi a distanza: i referti arrivano per email, i dati online, la consulenza in videochiamata. Vengo in studio solo chi vuole.

The whole path can take place remotely: reports by email, data online, counseling by video call. The practice is for those who want to come.

Quanto costa?How much does it cost?

La prima valutazione dei documenti è gratuita. La consulenza genetica costa 120 €. La rianalisi di un esoma già fatto parte da 450 €, un genoma da 650 €: i prezzi completi sono nella pagina della rianalisi. Per un esame nuovo, il costo dipende dall'esame e ve lo dico in consulenza.

The first assessment of the documents is free. Genetic counseling costs €120. Re-analysis of an exome already done starts at €450, a genome at €650: full prices are on the re-analysis page. For a new test, the cost depends on the test and I tell you during counseling.

I nostri dati sono al sicuro?Is our data safe?

I file vengono usati solo per la vostra analisi, trattati secondo il GDPR, e conservati solo per il tempo scritto nel consenso che firmate. Non finiscono in nessun database esterno senza il vostro permesso.

Files are used only for your analysis, handled under GDPR, and kept only for the time written in the consent you sign. They do not end up in any external database without your permission.
